Children’s Hospital Urgent Care will release medical records during operating hours to those who are authorized to receive a patient’s records. Completion of the Authorization for Release of Protected Health Information form and a government issued photo ID is required to be presented in person. Whenever possible, records should be requested from the office where care was provided.
Requests for records to be sent to a third party can be made by submission of a completed Authorization for Release of Protected Health Information and a scanned copy of a government issued photo ID of authorized parent/guardian. Requests can be made in person or required documents can be faxed or mailed to the site where care was provided.
