Ultimately, our patients are the reason Children’s Hospital Urgent Care exists. We constantly work to make your children’s urgent care experience a positive one. For immediate medical questions or concerns after regular business hours, please contact your Children’s Hospital Urgent Care location. For the safety of our patients, we cannot give medical advice of any kind via the internet or phone.
Blount Outpatient Center:
Children’s Hospital Urgent Care will release medical records during operating hours to those who are authorized to receive a patient’s records. Completion of the Authorization for Release of Protected Health Information form and a government issued photo ID is required to be presented in person. Whenever possible, records should be requested from the office where care was provided.
Requests for records to be sent to a third party can be made by submission of a completed Authorization for Release of Protected Health Information and a scanned copy of a government issued photo ID of authorized parent/guardian. Requests can be made in person or required documents can be faxed or mailed to the site where care was provided.
